This might be the best option you have to cancel your Kaiser insurance. You have to complete the form and give it to your immediate supervisor (in case you joined a program through your work).

On the other hand, if you joined on your own, just submit the form to your local Kaiser Permanent Office.

In the form, you will have to complete information related to:

  • Your contact identification: names, date of birth, phone number, social security number, and your address.
  • Identification of family members that may get affected when you cancel your insurance.
  • Your signature and date.
  • In case you have dependents that are 18 or older, they must sign as well.

There are other lines to be filled, but in this case (canceling), you only need to complete this specific information; you will also find instructions in the form.

  2. I didn’t find any option from my account to terminate my Kaiser health plan, and I called Covered California customer service to terminate the plan, however I didn’t receive any confirmation mailing letter.

  3. Hi..through my husband’s job he got insurance for the family..I didn’t need to be added. Since I already had my own health insurance. My problem is, how do I get Kaiser off my health insurance..it shows up as my primary health insurance. I recently moved locations and all my prescriptions, medical visits, testing, mental health has been paused. I been trying to get this handled since December 2020. Please help me. I’m completely worried about getting my medication for health reasons.

  4. Kaiser sent me to a snf which wont/can’t meet my onerous needs ie; pericare, wheelchair rental, etc…as I’m a t3 paraplegic, who recently had a left shoulder reverse surgery.. I also have dc medicaid which is excepted by N.R.H the hosp who cam provide the post-op care needed!!
    How do I use my dc medicaid or at least make medicaid my primary & kaiser my 2nd ins??
